Two AI systems — Mayo Clinic's REDMOD and Alibaba's PANDA — can detect pancreatic cancer on routine CT scans months to years before clinical diagnosis, with REDMOD identifying 73% of pre-clinical cases an average of 475 days early and PANDA achieving 92.9% sensitivity with 99.9% specificity. But the low prevalence of pancreatic cancer (about 15 per 100,000 annually), unresolved questions about false-positive harms, demographic generalizability, cost-effectiveness, and the absence of external replication outside originating institutions mean the path from promising study to population-wide screening remains long and uncertain.
